Fix the Problem, Not the Whole Ankle
Ankle Arthroscopy for Athletes and Active Adults
Some ankles don't need to be opened — they need to be cleaned out. Loose fragments that catch and lock. Bone spurs that pinch every squat and landing. Ankle arthroscopy fixes these mechanical problems through incisions about 3 millimeters wide, using a 2.4 mm camera — without creating a new surgical trauma on the way in.

What It Treats
The Mechanical Problems a Scope Solves
- Loose bodies — fragments of bone or cartilage floating in the joint, causing catching and locking
- Anterior impingement ("footballer's ankle") — bone spurs and scar tissue at the front of the joint from years of cutting and landing — my breakdown of Freddie Freeman's ankle arthroscopy explains this one
- Cartilage damage — smoothing frayed surfaces; full restoration for deeper lesions lives on the ankle cartilage repair page
- Synovitis and scar tissue — the inflamed lining that keeps an old sprain achy and swollen
- High ankle sprains — arthroscopy-assisted TightRope stabilization of the syndesmosis
I'm also one of the few surgeons in the region performing posterior ankle (hindfoot) endoscopy for problems in the back of the joint — covered on the ankle impingement page.
Honest Scoping
Not a Car Wash
Arthroscopy works when there's a mechanical problem to fix. If your problem won't benefit from a scope — like widespread arthritis — I'll tell you, and tell you what will help instead.
The Workup
Confirm First, Operate Second
Impingement tests, ligament testing, tendon assessment — locating the true pain generator.
Cartilage and soft tissue — what's frayed, what's loose, what's inflamed.
In-office standing CT defines subtle bony problems and alignment.
When the source is unclear, numbing the suspected spot gives proof before an operation.
The Comeback
Recovery: Faster, Not Careless
- Same-day surgery
Home within hours.
- Boot protection
Physical therapy typically begins early.
- Brace & build
Transition to a brace; strengthening builds.
- Progressive return
Return to running and sport — benchmarked, not guessed.
